PROGRAM FACILITY

 

DESCRIPTION OF PHYSICAL SITE

 

The facility is licensed to accommodate up to 150 individuals, including 25 (non-ambulatory) individuals who require the usage of assistive devices such as a wheelchair. It is a gated facility that requires a code for entry, but can be set to remain open throughout the duration of the day.

 

The program facility is situated close to Capitol Expressway in the east side of San Jose. The neighborhood that the program facility is settled in is in a safe, residential area where it is surrounded by three (3) local places of worship: a Buddhist temple, evangelical church and a Christian chapel.  It is in close proximity to other amenities such as Eastridge Mall, Lake Cunningham Park and recreation, Chase bank, restaurants/fast-food chains, elementary schools, Home Depot, and other retail stores.

 

Recreational parks, museums, schools and other that partnered with our program are within driving distance from the facility.

 

A. Location: 888 S Capitol Ave, San Jose, CA  95127

 

 

B. Size: 5,969 square feet of building space sitting on a 1.03-acre lot. The

property has 42 parking spaces to accommodate vehicles for the program, employees, and visitors. 1 space at the entrance of the parking lot is reserved for the “Employee of the Month”. The outdoor space provides plenty of opportunities for outdoor activities i.e. sports, exercises, and gardening.

 

C. Features: The facility is equipped with newly installed centralized HVAC system with HEPA (High Efficiency Particulate Air) filtration which purifies and filtrates up to 99.7% of all contaminants like dust, viruses, pollen, mold, etc. that helps in the mitigation of infection control. The facility has ten (10) toilets complete with wash basins and personal care rooms designed in strict compliance with the required city and fire safety codes. The administrative office is complete with computerized work stations and spaces for meetings and conferences.

 

D. Additional features of the training facility and office of the Apollo Adult Day Program that enhance supports, programs, and services, include but are not limited to the following:

 

 

  • Lobby/Reception Area: This area provides a welcoming environment with comfortable relaxation chairs that may be used by individuals who are waiting for a meeting, or their ride home. There are two administrative offices in this area, one with a small conference room that can accommodate 5 people, and another room that can accommodate 10 people in a meeting.

 

 

  • Multipurpose room that can be setup for trainings, music appreciation, multiple occasions (holidays, birthday parties, etc.) as well as provide activities that are preferred or chosen by individuals (i.e. bingo, games, talent shows, art expo, etc.). It is equipped with a 150” motorized projector screen for individuals who want to participate in our virtual classes, Zumba, chair yoga and physical fitness classes hosted by our own Fitness Training Coordinator.

 

  • Exercise room which are fully equipped with age-appropriate and skills-adaptive equipment that will provide opportunity for our individuals to participate in exercise routines and machines. Each machine comes with safety mechanics to reduce the potential for injury.

 

  • The facility has a computer area equipped with 6 new computers for our individuals to utilize to browse the internet, search/apply for jobs, hone their typing skills, shop online, attend zoom meetings, online trainings, and access appropriate websites and social media.

 

 

  • Restrooms which are all code-compliant for ambulatory and non-ambulatory individuals. There are (10) ten stalls accompanied by (10) ten washbasins.

 

  • Dedicated rooms such as the activity rooms that are fully stocked with emergency and first aid supplies and a locker room where participants and training assistants can leave their personal belongings when they go out in the community. Individuals’ medications will be stored in a lockable cabinet within this room along with the MAR (Medication Administration Record). This room has a sink that is dedicated for hand-washing trainings, self-help skills, etc.

 

 

  • Movie/AVS (Audio Visual Sensory) room that utilizes a high-powered projector as well as (10) reclinable theater seats to emulate a small theater akin to auditoriums in movie theaters such as AMC and Century. This provides individuals the opportunity to participate in a theater setting or host their own private watch party with their peers. The room may also be utilized as a stepping stone to assist individuals who have seldom or never been in a theater with the goal to help them adjust to this type of community setting. This room provides space for individuals who utilize wheelchairs to situate their assistive device in the front. It can accommodate up to 15 people.

 

 

  • Library room that can be utilized by Training Assistants and individuals to assist in the development, acquisition, and implementation of skills that pertain to life skills such as money management, resume creation, group and current events discussions, etc. All participants have the opportunity to read any book of their choosing from the room and may participate in reading sessions with their peers.

 

 

 

  • Kitchen gives our individuals plenty of opportunity to learn and to hone kitchen skills for simple cooking, baking, table setting, and other self-help skills with the assistance of the support staff. Kitchen is equipped with a refrigerator, touchless water dispenser with hot and cold selection and a safety feature for the hot water, microwave, and an electric stove. All sharp kitchen utensils will be stored in a locked drawer for safety purposes, and the knobs of the stove will be removed every time the stove is not in use. The kitchen sink is equipped with an enhanced filtration system that is designed to guarantee water safety and health to provide unlimited clean water by eliminating up to 99% of contaminants such as chlorine, tastes, odor, toxic fluoride, arsenic, lead, nitrates, and heavy metals.

 

 

  • Outdoor Patio/Lounge provides both staff and individuals a common space for them to have their lunch or to spend time during breaks. The space boasts a great open area with natural lighting from the ceiling panels above. The area is also equipped with 2 heaters for colder days akin to the outdoor dining experience.

 

 

E. Video Surveillance:    Apollo Adult Day Program will have twenty-four (24) operational non-audio video surveillance cameras both within and outside the facility, to ensure and promote safety for both training assistants and participants. All cameras will be installed only in common/public areas of the facility such as hallways and the common rooms. The application for the cameras is not accessible to the public, but only to staff that are part of management may access the video’s feed via a console located in a locked room. Management staff is required to input a password to enable them to log into the camera’s application via the console. Videos are recorded throughout the week and are set to automatically erase once every two (2) weeks at midnight (12:00AM) on Saturday.

 

All participants will have a consent form in regards to the cameras sent along with their acceptance packet to inform them, their conservators (if applicable), homes, Service Coordinator, and families of the use of video surveillance within the facility.
